Firstox accepts Medicare, Medicaid, and private insurance. If you are insured, Firstox will file an insurance claim on your behalf. Your insurer will process your claim and send you an explanation of benefits (EOB), which describes the services you received, how much your insurer paid for the services, and any charges that it did not cover. Your EOB is not a bill. Firstox will bill you for any applicable copayment, coinsurance, or deductible required by your insurer. These amounts are determined by your insurer, not Firstox.
If you do not have insurance or if you are insured but your health plan does not cover your test, Firstox may offer you an uninsured rate. Please contact our billing department for more information.
